Students interested in trying out for sports teams must submit the Annual Athletic Pre-Participation Physical Examination form, the Athlete Emergency Information Card and the Online Registration form.
Both the Pre-Participation Physical Examination form and the Athlete Emergency Information Card should be downloaded and completed prior to the scheduled tryout. Please make sure to turn this form into the Athletic Director.
The Athletic Registration form can be filled in and submitted online (please see below). The registration must be completed by both the parent and the student athlete. Once you begin the form, you will not be able to save and return at a later date. As a result, please ensure that both the parent and student are present.
All sections require the parent's digital signature as an indication of understanding and compliance with Athletic Department Policy. Sections directed specifically at students, require both the student's and parent's signature as a commitment to uphold all MHC Academy values and standards of conduct, as well as to comply with NJSIAA regulations.
The Registration Form will take approximately 15-30 minutes to complete and must be completed in one session.
The Athletic Online Registration Form must be completed online prior to tryouts. Please contact the Athletic Director with any questions you may have. Registration for 2024-2025 will open July 1, 2023.
